Application Procedures and Requirements

Step One: Submit the following requirements:
- Birth Certificate (2 photocopies)
- Baptismal Certificate (1 photocopy)
- Report cards of previous levels beginning Grade 7 (1 photocopy each)
- Certified True Copy of Current Report Card
- Accomplished Student Information Sheet (Download form from STCQC website)
- Accomplished Recommendation Form (Download form from STCQC website)
- Certificate of Good Moral Character from the child’s current school
- 1 x 1 colored ID pictures (2 pcs.)
- Alien’s Certificate of Registration (1 photocopy) – For foreign applicants only
- Passport Data Page (1 photocopy) – For foreign applicants only
For online submission: email to registrar@stcqc.edu.ph
For on-site submission: submit to the Registrar’s Office (enter via Gate 1) Office Hours: 8:00am to 12:00nn/1:00pm to 3:00pm
Lunch Break: 12:00nn to 1:00pm
NOTE: for on-site submission, bring original and photocopies of Birth and Baptismal Certificates
Step Two: Wait for an email from the admissions officer once the documents have been submitted.
